اولاً : متى تلقيت المريض ؟ تاريخ كم ؟ الساعة كم ؟ نسبة الوعي عند المريض GCS ؟

ثانياً : اي حاجة مُتركبه على المريض تكتبها زي cannula, Nasogastric tube, urinary catheter
وتكتب كل التفاصيل له

ثالثاً : اي ملاحظات على المريض

رابعاً : الاكشنز اللي سويتها للمريض
for example : ECG, blood transfusion, checked vital signs and records , give patient medication

خامساً : الrecommendation توصّي الnurse اللي بيمسك الحالة بالshift اللي بعدك وغالباً إذا الحالة مُستقرة تكتب continues of care
