DEFINITION
Restrain is involuntaryu use of physical or mechanical device to limit or restrict movement of the whole or portion of the patient’s body as a means of controlling his or her physical activities

Restrain are to be used only during emergency situations when there is immenent risk ofr individual harm or others

PROCEDURES
•Initial assessment should be done by healthcare professionals. Restrain shall be limited to preventing and reducing serious, destructive/damaging actions by patients such as theathening self, to other patients or to staff

•attempt a less restrictive restrain first, should be reviewed every 24 hours then assessment shall be done if to continue or discontinue.

can apply then must inform immediately or within an hour and must take an order from the physician. Verbal orders fro restrain shall be signed by physician within 24 hours
If the initiation of restrain is based on significant change in patient’s condition the RN must notify physician immediately

•Patient shall be reassessed hourly to determine adjustments if necessary

•If chemical restrains are used, vital signs are closely observed and monitored. If hypotension occurs, stop sedations if infusion is ongoing

Restraints should be released every 2 hours with some form of activity including range of motion , repositioning, ambulation and or toileting. Release of restraints and the intervention provided must be documented

•Patient and family shall be educated for reason for restraint, how patient and family can avoid restraint and criteria necessary for restraint
•Re-assessment of Use: the physician in collaboration with HCT shall evaluate patient at the end of the prescribed duration of restrain to determine the need for continued use of the device, if same is necessary the order must be renewed.
If there is no renewal order the restraint shall be removed by the responsible nurse or the caregiver
