Iron deficiency anemia:
It is a condition in which blood lacks adequate healthy red blood cells. Red blood cells carry oxygen to the body's tissues !
Etiology:
🔺Inadequate dietary intake like being a vegetarian or low food intake of sources that contain iron.
🔺Increased need to support increased blood volume like during pregnancy and lactation as well as for adolescents and infants.

🔺Blood loss or increased excretion like after surgery or when you have wounds or ulcers.
🔺Impaired iron absorption like when you suffer from achlorhydria or GI diseases
🔺Iron present but not well used like in cancer patients or when you have infections.

Basic screening Tests:
Red blood count, hematocrit, hemogobin, red blood cell indices: MCV, MCH, MCHC
White blood count, and platelet count. ✔️

Diagnosis:
🔺serum ferritin (most sensitive): normal is > 12 ng/dI
🔺Transferrin: normal is 200-400 mg/dI
🔺serum iron: normal is 50-175 mg/dI
🔺transferrin saturation should be > or equal to 16%
🔺hemoglobin: female 12-16 g/dI, male 13.5-18 g/dI

Treatment:
Treat the underlying disease**
Oral elemental iron in ferrous form*
50-100 mg 3 times daily for adults
4-6 mg/kg of body weight divided into 3 doses/day
Take it with vitamin C to better absorb it *
Enhance the absorption of non-heme iron with ascorbic acid

Response to treatment: استجابة المريض للعلاج
Week 2: Reticulocytosis (increase in young RBC)
Week 3: Increased Hemoglobin halfway to normal
Week 8: Normal Hemoglobin
Continue Ferrous Sulfate 325 mg PO for 4-6 months

Medical nutrition therapy for iron deficiency: فقرة الدايتشنز 😍
Good source of iron contains > 10% of the US RDA
You need to absorb 1.8 mg iron/day
Try to take iron with vitamin C
(Keep in mind, the lower the Fe stores, the greater the rate of absorption)

Cont..
Heme sources like (meat, poultry, fish); about 15% absorbable
Non-heme sources (grains, vegetables, eggs); about 3% to 8% absorbable
لاحظتوا كيف الفرق بين امتصاص الجسم للحديد من المصادر الحيوانيه و النباتيه؟ 🤔
