مساء الخير ي أصدقاء 🙋🏻‍♂️
نستكمل موضوع أمس ، وبذكر أنواع veins بشكل مختصر لأهميتها
هناك نوعين بشكل عام وكما يعرف الجميع 🗣
وموجوده في التغريده الأولى 👀
- Superficial veins
- Deep veins
بعد ماعرفنا النوعين .. نبدأ في موضوعنا لهذا اليوم وهو :
( Venous Thromboembolic Disease )

طيب نفصل في العنوان شوي ونكون فاهمين ايش معنى ؟🤔
Thrombo
جايه من كلمة Thrombus طيب ايش معناها:
هي عباره عن:
A blood clot that forms on the wall of a blood vessel
طيب وايش معنى؟
Embolic
جايه من كلمة Embolus وهي عباره عن :
A blockage or plug that obstructs blood vessel

بعد ماعرفنا المعنى اللغوي لهذا المرض ندخل في أقسامه :
هذا المرض يشمل شيئين لا ثالث لهما ..
الأول :
Deep venous thrombosis (DVT)-
والثاني :
Pulmonary embolism (PE)-

ايش الفرق بين DVT و PE ؟
DVT :
يصير في Deep veins✔️
وليس في Superficial veins❌
و هو عباره عن تجمع لل clot وين ؟من اسمها في
Deep vein of the lower extremity or pelvis
بس تصير في Lower Limp ي عامر ؟(لا) ي الحبيب ممكن تصيب برضه upper limp مثل Arms
But most common in the LL 🔴

طيب ايش هي PE ؟ 🙄
هي عباره برضوا عن تجمع لل clot طيب وين يصير ؟
في lungs . 🧐

و طريقة diagnosis
و TTT

قبل ماأتكلم عنsign and symptoms ودي أتكلم عن مراحل تطور DVT 🗣
🔝إتجاه السهم يدل على Blood flow🩸
نبدأ من(A)
يوجد على الجانب Thrombus ولكنها لاتمنع حركة الدم في vein .
بمعنى آخر:
Thrombus in valve pocket of a deep vein with blood flowing beside thrombus

( B ) : Thrombi tend to form at bifurcations of deep veins
و لكن يوجد بطئ في حركة وتدفق الدم
( C ) : ( Complete occlusion )of vein by thrombus forcing backflow of blood.

(D):Embolus that has broken off from a thrombus and is floating in bloodstream could migrate to lungs and cause( pulmonary embolus ) .

خلونا نشوف مع بعض ايش هوVTEعن طريق ذكر بعض الأرقام والحقائق وايش تقول الأبحاث💡

اولاً :
Venous thromboembolism (VTE ) is a serious condition, with an incidence of 10% to 30% of people dying within ( 1 month of diagnosis )😳 , and half of those diagnosed with a DVT have long-term complications.
طبعاً مثل ماذكرت سابقا
VTE including DVT and PE .👀

ثانياً :
One third of individuals will experience another VTE within 10 years .🙄

ثالثاً :
VTE ممكن تحدث بعد العمليات الجراحيه ك complication 👀 مثل
(TKR) Total knee replacement وهذه اكثر عمليه ممكن يصير معها هذا المرض .
The incidence of DVT after TKA is 40% to 80% and
The incidence of PE is approximately 2%.

رابعاً وأخيراً :
The a long term complications of VTE are post-thrombotic syndrome (PTS), which occurs in 20 to 50 % of patients with DVT
And chronic thromboembolic pulmonary hypertension (CTEPH), which occurs
in 2 to 4 % of patients with PE

Signs and symptoms of a PE are:
1️⃣shortness of breath
2️⃣decreased SpO2
3️⃣pleuritic chest pain
4️⃣cough
5️⃣and tachycardia
طريقة التشخيص ؟

1️⃣ventilation/perfusion scan or
2️⃣computed tomography(CT)pulmonary angiogram or
3️⃣D-dimer test .

Signs and symptoms of a DVT :
- Pain ( is often described as being a cramp or ache in the calf or thigh )
- redness .
- swelling .
- warmth or erythema.
طريقة التشخيص ؟

- Two-level DVT wells score .
