Acute Kidney Injury (AKI):
Thread on guide to Diagnosis and Management
Throughout this thread we will review some developments about the Dx and Tx of AKI
Some contraversial points also will be discussed inshaallah 🤍🍃
Definition of AKI: is an abrupt loss of Kidney function +/- oliguria +/- anuria.
The resulting effect include:
👇🏻👇🏻👇🏻
Remember🔎: AKI is usually Reversible decline in the eGFR 🤲🏻
What is important to know that our precious organ (kidneys) received 25% of our cardiac output ♥️ and that our lovely filtring units (Nephrons) filters approximately 180L’s water 💦 daily without a stop 🛑 . Almost (125 mL / min ) of our plasma 🧪
📝Note:
The normal value for eGFR is influenced by many factors
1- age
2- muscle mass (body size)
3- gender (👨🏻, 👩🏻)
4- Race
📝Note:
The normal eGFR value varies between 👩🏻/👨🏻, for the previous reason related to muscle mass/body size
📝Note:
No EXACT eGFR can be calculated
What is important to know as a practitioner is wheathr the estimated Glomerular filtration Rate is Changing 😦 or stable 😃
Ext Point Is Very Important: 🚨
The difference between the following terminologies
👇🏻👇🏻👇🏻
Acute Kidney Disease /AKD
Acute Kidney Injury / AKI
Acute Renal Failure / ARF
Are they interchangable ?
Which one is the most accurate to use when you want to describe ur patient 🔎
The term AKI ✅ (ACUTE KIDNEY INJURY) replaced the previously used ARF ( acute renal failure) ❌
Why?
Bcuz the smaller changes ↕️ in kidney function without evident FAILURE can result in 🛑 SIGNIFICANT Clinical Consequences ✋🏻 then ⬆️ mortality 💀⬆️morbidity
REMEMBER 💡
AKI = ⏰⬆️7️⃣Days
⬇️3️⃣Months
The duration of injury is not included in the definition , but has been shown to be related to prognosis 🚦🥊🏆🎭
AKI & CKD often form a continium of disease! they are not a separate conditions
Many disease midifiers & risk factors may represent a chance to mediate the poor outcomes that usually associated with AKI
Disease modifiers , Risk Factors , poor outcomes 👇🏻👇🏻👇🏻
Important TimeLine 😃 for days post any injury
Worth to remember 👩🏻‍🏫 👍🏻
⏰AKI= 48Hr —> ⬇️7️⃣days
⏰AKD=⬆️7️⃣days -> ⬇️3️⃣month
⏰CKD 💀 = ⬆️3️⃣month &continue
Remember 💡
Recovery from AKI within 48Hrs of the initial injury represent
RAPID REVERSAL 💃🏻 💃🏻 🏆✋🏻
Patients who suffer AKD<< with preexisting 🥊 CKD 🥊 are at HIGH RISK of Kidney Disease PROGRESSION 💀
Disease Epidemiology
ATN accounts in ICU patients is caused by Sepsis in 35%-50% of cases
ATN = Acute Tubular Necrosis
ICU = Higher Mortality usually ⌛️⏳
