Will keep the learning process in ICU rolling! Tonight I’ll write a thread about top 20 important critical care trials in my opinion that any interns/ residents should understand before finishing ICU rotation????#FOAMed #SCCM #ICU
Few weeks ago, I got inspired by @Paul_Wischmeyer for the top 12 ICU trials. In this thread I’ll highlight the top 20 ICU trials from my point of view as critical care Pharmacist. Results of trials might not reflect my agreement with it in my practice
1/SAFE trial (NEJM 2004), Albumin Vs. Saline for resuscitation in the ICU.
2/CRASH trial ( Lancet 2004), IV corticosteroids use in significant head injury
3/NICE-SUGAR trial (NEJM 2009), intensive Vs. conventional glycemic control in ICU
4/ CORTICUS trial (NEJM 2008), hydrocortisone therapy for septic shock
5/ River trial (NEJM 2001), early goal directed therapy for sepsis and septic shock
6/ Leuven trial (NEJM 2001), intensive insulin therapy in ICU
7/ Van den berghe trial (NEJM 2006), intensive insulin therapy in the medical ICU
8/ SEDCOM trial (J of Am Med Association 2009), dexmedetomidine Vs. midazolam for sedation in ICU
9/ MIDEX, PRODEX trial (JAMA 2012), dexmedetomidine Vs. midazolam or propofol for sedation in MV
10/ MENDS trial (JAMA 2007), effect of sedation with dexmedetomidine Vs. Lorazepam in MV patients

11/APROCCHSS (NEJM 2018), hydrocortisone plus fludrocortisone for septic shock
12/ ADRENAL (NEJM 2018), adjunctive glucocorticoids in septic shock
13/ SMART trial (NEJM 2018), balanced crystalloids Vs. saline in critically ill patients
14/ SALT trial (Am j Respir Crit Carr Med), balanced crystalloids Vs. saline in ICU
15/ SPLIT trial (JAMA), buffered crystalloids Vs. saline effect on AKI in ICU patients
16/ SPICE trial (NEJM 2019), early sedation with dexmedetomidine in critically ill patients
17/ MIND trial (crit care med 2010), use of antipsychotics for ICU delirium
18/ Marik et al (Chest 2017), hydrocortisone, vit C and thiamine for sepsis and septic shock
19/ HOPE-ICU (Lancet 2013), IV haloperidol effect on the ICU delirium duration
20/ STOP-IT (NEJM 2015), short course of antimicrobial therapy for intraabdominal infection
The above mentioned trials, either have results that change the practice or introduce points of controversy in critical care practice. I believe knowing these trials along Sepsis and PADIS guidelines before finishing ICU rotation is essential! #FOAMed #SCCM
