A 90-year-old man 👨🏻🦳was admitted after a house fire. Physical examination revealed burns and soot all over his face. Laboratory tests showed blood pH 7.2 and carboxyhemoglobin 23.4% (reference interval 0 –1.5%), consistent with smoke inhalation.

Considering his clinical features and laboratory results, the diagnosis was carbon monoxide and cyanide intoxication. He was treated with hydroxycobalamin injection (Cyanokit®) and hyperbaric chamber sessions and recovered rapidly.
